It is 2 a.m.

But tonight, after a 12-hour shift, she is counting money.

After sending RM800 back to her parents in Kedah, how much is left to buy school shoes for her daughter?

Earlier, her manager told her she is “not senior enough” to handle VIP clients — even when customers request her by name. A RM150 booking was given to a senior therapist who has not updated her skills since 1998.

She receives only RM50 per client.

Her body aches — but she dares not take medical leave. “No replacement available,” they said.

This is the silent reality of 50,000 talented massage therapists in Malaysia today.


The World Customers Never See

You enter a beautiful spa. Soft music. Lavender scent. Warm lights. A smiling therapist welcomes you.

After 90 minutes, your pain disappears. Your stress melts away. You pay RM180, tip RM20, and leave feeling renewed.

But what you never see:

  • The therapist still has to mop the floor

  • Sanitize the room

  • Wash and fold towels

  • Prepare for the next client

From your RM180 payment, she receives only about RM50.

She has been standing for 11 hours. Her own back hurts — yet she cannot afford a session for herself, because one massage equals two days’ wages.

She learned new techniques from YouTube — but cannot apply them because, “This spa doesn’t do that.”

And every time an enforcement uniform appears, her heart races — because she knows she does not have an official certificate or the Ministry of Health Annual Practising Certificate (APC).

Not because she lacks skill.
But because the system never provided a fair pathway.


The Five Cages Trapping Malaysia’s Therapists

1. Certification Cage

  • Many learn through traditional apprenticeship

  • Official certification costs RM5,000–RM15,000 and 6–12 months full-time study

  • No certificate → no APC → legal risk

  • Enforcement is increasing

2. Economic Cage

  • Average pay: RM50–80 per client

  • 10–12 hours a day, 26 days a month

  • No EPF, SOCSO, medical leave, or annual leave

  • Monthly income: RM2,500–RM4,500

  • Certified therapists overseas earn RM11,000–RM19,000 monthly

3. Identity Cage

  • Seen as “spa workers,” not healthcare professionals

  • No career pathway

  • Skills undervalued

  • Cannot legally open their own practice

4. Time Cage

  • “No replacement” culture

  • Guilt used as control

  • No real rest

  • Time belongs to the business, not to themselves

5. Fear Cage

  • Fear of raids

  • Fear of fines up to RM50,000

  • Fear of being reported

  • Fear of sudden collapse of livelihood
