- Titration: Increment by 100 /mg every 2 /weeks, based on clinical response and tolerability.
- Typical maintenance range: 100–400 /mg/day (often split into 1–2 doses).
- Max dose: Up to 600 /mg/day in divided doses, though doses above 400 /mg/day offer little additional benefit and carry higher risk of side effects.
- Pediatric & adolescents (≥6 /years): Weight-based start at ~1 /mg/kg/day, titrated weekly or biweekly, not to exceed 500 /mg/day.
- Special populations: Use caution and slower titration in patients with renal or hepatic impairment. In severe organ dysfunction, use may be limited or contraindicated.
Administration Instructions:
- Route: Oral—swallow capsules or tablets whole; do not chew or crush.
- With or without food: Can be taken either way; food does not significantly affect bioavailability, though it may alter absorption rate.
- Consistency: Take at fixed time(s) each day to maintain steady blood levels and efficacy.
- Missed dose: Skip if close to next scheduled dose—do not double up.
- Tapering off: Must be withdrawn gradually under medical supervision—abrupt cessation can lead to increased seizure frequency or other risks
