This inspection checklist is designed to align with ANSI/ISEA Z358.1 standards. You can print this out or digitize it for your safety officers to use during their rounds.

1. Access and Visibility (The 10-Second Rule)

• [ ] Path is Clear: No pallets, trash cans, or equipment obstructing the path to the station.
• [ ] Distance: The station is reachable within 10 seconds (approx. 55 feet) from the hazard.
• [ ] Signage: A highly visible safety sign is posted above or near the unit.
• [ ] Lighting: The area is well-lit so the station can be seen even if room lights fail.
• [ ] No Doors: There are no more than one door to pass through, and it must open in the direction of travel.

2. Operational Performance (The ''Bump Test'')

• [ ] One-Second Activation: The valve goes from ''off'' to ''on'' in one second or less.
• [ ] Hands-Free: Once activated, the water stays on without the user holding the handle/lever.
• [ ] Shower Flow: The shower head provides a minimum of 20 gallons per minute (GPM).
• [ ] Eyewash Flow: The eyewash provides a minimum of 0.4 GPM (or 3.0 GPM for Eye/Face washes).
• [ ] Spray Pattern: Water covers both eyes simultaneously and is not at a velocity that could injure the eyes.
• [ ] Height: Eyewash nozzles are between 33 and 53 inches from the floor.

3. Water Quality and Temperature

• [ ] Tepid Water: Water temperature feels lukewarm (between $16 \text{--} 38^\circ\text$ or $60 \text{--} 100^\circ\text$).
• [ ] Clarity: Water runs clear (no rust, sediment, or ''rotten egg'' smell from stagnant pipes).
• [ ] Drainage: Floor drains (if present) are clear and not backed up.
• [ ] Protective Covers: Dust covers on eyewash nozzles are in place and pop off automatically when water is turned on.

4. Self-Contained/Portable Units (If Applicable)

• [ ] Fluid Level: Tank is full to the manufacturer’s line.
• [ ] Expiration Date: The flushing fluid or preservative is not expired.
• [ ] Tank Integrity: No cracks, leaks, or signs of mold growth in the reservoir.
