Shoulder impingement occurs when the tendons of the rotator cuff (a group of muscles and tendons that stabilize the shoulder) become compressed or “pinched” between the upper arm bone (humerus) and the bony roof of the shoulder (acromion).
Rotator cuff issues range from tendon irritation and inflammation (tendinitis) to partial or complete tendon tears. These problems often overlap—ongoing impingement can lead to rotator cuff damage, and weakened rotator cuff tendons can worsen impingement.
Common symptoms include:
Shoulder pain, especially when lifting the arm overhead or reaching behind the back
Pain at night, often worse when lying on the affected shoulder
Weakness in the shoulder or arm
Limited range of motion or stiffness
Sharp pain or catching sensation during certain movements
Gradual onset of pain that worsens over time (sometimes mistaken for a minor strain)
These conditions can develop due to one or more of the following:
Repetitive overhead activities (e.g., sports like swimming or tennis, or jobs involving lifting)
Poor posture, such as rounded shoulders or forward head position
Muscle imbalance or weakness, especially in the rotator cuff or shoulder blade muscles
Age-related wear and tear of tendons
Bone spurs on the acromion that reduce space in the shoulder
Previous shoulder injuries or trauma
Improper exercise technique or sudden increase in activity
If left untreated, these conditions can progressively worsen:
Chronic pain and persistent inflammation
Reduced shoulder strength and mobility, affecting daily activities
Partial tears progressing to full rotator cuff tears
Permanent loss of function in severe cases
Increased likelihood of surgery instead of conservative treatment
Longer recovery time and higher risk of re-injury
Early recognition and management—such as activity modification, physiotherapy, posture correction, and strengthening—can often prevent long-term damage and restore normal shoulder function.
